If you or someone you support needs clear, defensible evidence for funding, plan reviews, or changes in support needs, a Functional Capacity Assessment (FCA) helps show how daily functioning is impacted and what supports are needed to improve participation and quality of life.

At Constellation Therapies, our FCAs are designed to be thorough, practical, and easy for decision-makers to understand, whether the participant is a child, adult, or receiving Support at Home services.

  • We complete Functional Capacity Assessments for:

    • Children and adolescents — home, school, emotional regulation, sensory needs, daily living

    • Adults — independence, executive functioning, work capacity, psychosocial and functional impact

    • Older adults / Support at Home participants — safety, independence, functional decline, daily living support needs

    • NDIS access evidence

    • NDIS plan reviews or change of circumstances

    • Support justification (therapy, support workers, home supports, equipment)

    • Documenting functional impact across home, education, work, and community

    • Supporting transitions across life stages

  • From beginning to end, we aim to have your Functional Capacity Assessment completed within 4 weeks.

  • Assessment fee: $2521.87

    May be covered by:

    • NDIS

    • Medicare (where applicable)

    • Support at Home (where applicable)

    Payment plans available for privately funded clients.

Referral or Enquiry

We start by confirming suitability, funding pathway, and urgency. We guide you through next steps and what information or documentation will help make the assessment as strong and meaningful as possible.

Information Gathering

We review relevant plans, previous reports, and background information. This helps us ensure the assessment is targeted, efficient, and reflects real day-to-day functioning across environments.

Assessment

The functional assessment is tailored to the participant’s age, needs, and environments, focusing on real-world functioning across areas such as daily living, regulation, communication, cognition, participation, &independence.

Report and Recommendations

You receive clear, comprehensive documentation outlining functional strengths, challenges, participation impacts, and practical support recommendations aligned to funding and real-life outcomes.

Post-Report Support

If ongoing supports are recommended, we can help match participants with appropriate therapies and services, including counselling, play therapy, occupational therapy, speech therapy, psychology, and capacity-building supports.
